NTSB: Loose Rivets Found in Plane With Hole

WASHINGTON -- Investigators trying to determine why the roof of a Southwest Airlines jet cracked open in flight have issued preliminary findings suggesting there may have been flaws in the riveting work when Boeing built the plane 15 years ago. The National Transportation Safety Board said Monday that some of the rivets used to bind the Boeing 737's aluminum panels together were sunk in holes larger than the rivet shafts. The holes weren't lined up correctly and were misshapen, not round, the board said. It didn't offer any conclusions and said the investigation is continuing. Southwest ...

Michelle Obama's Plane Flew Closer to Big Jet

WASHINGTON -- A plane carrying first lady Michelle Obama this week came even closer to a big military cargo jet than previously reported, the National Transportation Safety Board said Friday. The distance between the two planes closed to 2.94 miles before air traffic controllers at Andrews Air Force Base outside Washington directed the first lady's plane to abort a landing, the board said in a statement. The Federal Aviation Administration had previously said there was more than three miles between the planes, and had ordered controllers' supervisors to oversee flights carrying the first lady ...

NTSB: Cracks Found in 3 More Southwest Planes

YUMA, Ariz. - Inspectors have found small, subsurface cracks in three more Southwest Airlines planes that are similar to those suspected of causing a jetliner to lose pressure and make a harrowing emergency landing in Arizona, a federal investigator said Sunday. Southwest said in statement that two of its Boeing 737-700s had cracks and will be evaluated and repaired before they are returned to service. A National Transportation Safety Board member told The Associated Press later Sunday that a third plane had been found with cracks developing. Christine Ziegler, AP A Southwest ...

4 Dead in Crash of Small Plane at New Mexico Airport

ROSWELL, N.M. - Four Gulfstream employees were killed Saturday in a fiery crash of a test twin-engine luxury business aircraft at an airport, authorities said. Officials with Savannah-Ga.-based plane company confirmed that two Gulfstream pilots and two flight-test engineers died in the crash. Their names and other background information weren't immediately released. The company said in a statement that the Gulfstream G650 was doing takeoff-performance tests at Roswell International Air Center at the time of the crash. Lynn Lunsford, a spokesman for the Federal Aviation Administration, said ...

NY Police Crack Down on Discount Buses; Poor Safety Records Cited

New York police took a half-dozen tour buses off the road for violations as investigators probed the cause of two fatal crashes involving discount coaches and federal officials interviewed the driver involved in a weekend wreck that killed 15 people. Police on Tuesday set up a checkpoint in Chinatown, where the buses depart, and cited six buses for violations, including inadequate air brake pressure, steering problems and missing paperwork, The Wall Street Journal reported. Officers issued bus operators 40 summonses in all. Authorities are focusing on the buses in the wake of an accident ...

Could a Pipeline Explode in Your Community?

The apparent gas line explosion in Allentown, Pa., that killed five people is certain to refocus attention to the 2 million miles of natural gas pipelines that lie under neighborhoods and subdivisions from coast to coast. The Allentown explosion came three weeks after a gas main explosion in Philadelphia killed one person and gas pressure buildup caused an explosion and about 15 fires in Fairport Harbor, Ohio, about 30 miles northeast of Cleveland. The National Transportation Safety Board also will convene a public hearing March 1 into the explosion of a gas transmission line in San Bruno, ...

Mexican Drone Crash Lands in El Paso: 4 Possible Explanations

(Dec. 17) -- Someone needs to explain this. U.S. authorities are investigating an unmanned Mexican drone that crash landed in El Paso, Texas, on Tuesday, the El Paso Times reported Friday. Authorities identified the drone -- which landed in "someone's backyard" -- as an Orbiter Mini UAV designed by Aeronautics Defense Systems. The company's website describes the aircraft as "the ultimate solution for over-the-hill reconnaissance missions, low-intensity conflicts and urban warfare operations." Initial reporting on the drone crash was sketchy to say the least. "I guess we are investigating, ...

Landing Gear Glitch Forces Another Jetliner Down

Landing Gear Glitch Forces Another Jetliner Down

(Sept. 29) -- The same landing gear glitch that forced an airliner to make a miracle landing in New York last weekend has prompted another emergency landing in Milwaukee, and federal regulators are investigating the mishaps. Montreal-based Bombardier Inc. built the CRJ twin-engine jetliner that made an emergency landing Saturday at JFK airport after its landing gear malfunctioned. An announcement in the cabin to "brace for impact" could be heard on video taken by a passenger and sparks could be seen flying from the plane's wing as it dragged along the tarmac, but none of the 64 people aboard ...

After Gas Blast, Spotlight Shines on Pipelines

(Sept. 10) -- Damaged pieces of 30-inch steel pipe pulled from the ashes may tell the tale of what happened in Crestmoor Canyon on Thursday night, when gas from a ruptured transmission pipeline fed a fire so intense it devoured 15 acres of houses in the San Francisco suburb of San Bruno, Calif., and killed four people. What's certain is that the accident, coming on the heels of the BP disaster in the Gulf of Mexico, will once again cast a spotlight on the effectiveness of state and federal safety regulations governing the energy industry. Regulations passed by Congress in 2002 require safety ...

Everyday Miracles: Victims of Most Air Crashes Survive

(Aug. 12) -- It was just after 5:30 p.m. in the skies near Iliamna, Alaska, and the Cessna U206G airplane carrying a pilot and five sightseers was in trouble. The small propeller plane, now the subject of an investigation by the National Transportation Safety Board, had just taken off for its flight back to Anchorage from southeastern Alaska, but wasn't going to make it. The pilot reported the plane, which had taken on 42 gallons of aviation fuel before takeoff, had lost power. The Cessna was equipped with flotation devices for water landings, but the pilot had to bring it down onto ...
